Initially established as Roger Williams Medical Center Home Care in 1987, fully licensed, Medicare and Medicaid-certified, home health agency. Description
CharterCare will assign a specially trained Rhode Island certified nursing assistant (CNA) to help with personal care. Working under the direct supervision of a registered nurse (RN) or rehabilitation specialist, CNAs may assist with or perform a variety of supportive and personal care functions including but not limited to the following: Bathing and grooming Oral hygiene Hair care Shaving *Physician-ordered exercise Ambulation Encourage compliance with the rehabilitation plan *All exercise programs are taught and overseen by a registered rehabilitation specialist

Description
CharterCARE visiting nurse service is unique in that the clinical staff practices primary nursing. They will systematically assess, plan, deliver, and continually evaluate an individualized plan of care for each patient. Skilled Care: Clinical staff provides skilled training in the use, care and management of various medical devices including; G-tubes, J-tubes and foley catheters. We provide training in and assistance with injection therapy, oxygen therapy, ostomy care, tracheostomy care, the administration of insulin for diabetics, medication management, disease management and wound care. Advance Wound Care Every one of our nurses has been trained and is proficient in performing negative pressure wound therapy, a state of the art technology proven to significantly reduce healing time for chronic wounds. (ie: Wound Vac, Renosys and Engenex). Telemonitoring Our telemonitoring program is an adjunct to nursing visits and physician care. Upon the physician order, equipment is installed in the home for the patient. The equipment allows the patient to take daily measurements of weight, blood pressure, pulse and oxygen saturation. Results are monitored daily by an RN. Nutritional Services CharterCARE Home Health utilizes registered dieticians (RDs), food and nutrition experts who have met American Dietetic Association (ADA) criteria to earn the RD credential. RDs are available to assess a patient's nutritional needs, develop and implement nutrition programs, and evaluate and report the results.

Occupational Therapy The goal of occupational therapy is to help the patient regain maximum use of his/her upper extremities by improving arm strength, upper body flexibility and hand coordination. Our therapists have experience working with repetitive stress injuries, stroke limitations, and a variety of chronic conditions including arthritis, Alzheimer's disease, and multiple sclerosis.

Physical Therapy The goal of physical therapy is to restore the patient to his/her maximum physical strength, flexibility, coordination, and independence. Our registered PTs begin by assessing the patient's physical functions and mobility and consulting with the physician to develop personalized treatment plans. Rehabilitative services are available for post-operative joint replacements, musculoskeletal trauma, orthopedic problems, and neurological disorders.

Speech Therapy Speech therapy is prescribed to help patients who are experiencing difficulty speaking, communicating their thoughts, or swallowing. Our speech pathologists often treat patients with neurodegenerative diseases, memory and movement disorders, traumatic brain injury and stroke limitations. They begin by assessing the patient's speech, hearing, language, and swallowing abilities and then plan, implement, and evaluate a course of therapy.
