Description

Grief and bereavement support groups are held at various times and locations throughout the year to support individuals and families who have lost a loved one. Coming together with others to share similar feelings and thoughts is beneficial to the grieving process. The support groups are a 6-session series led by the hospice chaplain, who teaches healing and coping strategies. Other programs offered for families and friends who have lost a loved one include: Circle of Love and Light held in the first week of December. Annual Interfaith Service, held each spring.

      Skilled Home Nursing Care: Under the direction of a physician, skilled nurses work closely with patients and families to meet their physical, mental and emotional needs throughout Rhode Island. Nurses can: Assess patient needs; Teach patients and family members about their medications, disease processes, and appropriate treatment; Help assess wound healing and coordinate quality care to optimize healing; Conduct post-operative assessments and monitor patient’s progress and needs. For certain diagnoses, technology such as Telehealth can monitor weight, blood pressure, pulse and oxygen levels on a daily basis, alerting the nurse for any needed intervention to prevent hospitalization.

            VNA of Care New England's physical therapists help patients restore movement and function, ease pain, and increase strength and mobility. Using a wide range of techniques, they work in the patient's home to evaluate and treat problems with moving, balance, strength, and conditioning. This may involve retraining skills that the patient had before an illness or teaching new skills such as using a walker or cane. The physical therapists will help develop an individualized exercise program to help patients achieve their goals. Physical therapists also focus on fall prevention and home safety to allow patients to remain at home, with each care plan designed with the patient’s unique home environment in mind. Our physical therapists help with: Injury and illness. Stroke. Neuromuscular diseases including Parkinson's. Brain injury. Surgery. Neurological diagnosis.

              Speech therapists work with patients to correct speech or swallowing disorders and help restore language and communication skills. Many of our patients come to us after a brain injury, neuromuscular diseases, stroke, or other injury or illness. Based on each patient's unique needs and goals, the speech pathologist will develop a program with exercises and activities to strengthen muscles, sharpen speech, and improve reading, recall, and listening skills. Based on each patient's unique needs, the speech therapist will develop a program with the exercises and activities to meet patient goals. Our speech therapists also work with cognitive communication impairments such as attention, memory, and problem-solving disorders. Our speech therapists help patients: Improve clarity of speech. Improve reading, communication, and language skills. Understand safe swallowing strategies. Participate in LSVT LOUD treatment for individuals with Parkinson’s disease.
