HomMed Home Telemonitoring: Technology that monitors vital signs daily while at home and automatically transmits them to the South County Home Health office where a registered nurse reviews them.

First Connections services high risk moms and their children from birth to age three. Families are identified through the birthing hospitals as high risk and in need of community support. The intent is to reach all new moms within seven days after delivery or within 24 hours if the mom is !€œHigh Priority!€Pregnant women.

Healthy Families America is a national evidence-based maternal and child home visiting service. HFA pairs expectant mothers or families with newborns with a Family Support Worker, who provides free home visiting services to the family and their child until the child!€™s fourth birthday.

Provides free, voluntary, confidential home visits to pregnant women and families with young children, birth to age three. Trained nurses, social workers, and community health workers meet with families in their homes to talk about their needs, answer questions they may have, and conduct comprehensive assessments. Home visits are based on the needs of the family. Visit topics may include health education and connections with appropriate healthcare services, human services, and community resources. If a child does not have a medical home, we can help the family find one.

                        Description

                        Speech therapy involves relearning or improving your ability to form words, and it can help people of all ages. It can also help you: Understand and process what is being said. Find other ways to communicate after a stroke, including making gestures or facial expressions. Read, write, perform calculations, and remember things. Use strategies to cope when communication is difficult or problematic.

                        Office: Mon-Fri 8:00AM-5:00PM; Sat-Sun 8:30AM-4:30PM; Services 24/7 (on call coverage after office hours)
