Household income at or below 135% of federal guidelines, or rligibility in Temporary Assistance for Needy Families (TANF), Supplemental Nutrition Assistance Program (SNAP), Home Relief, Home Energy Assistance Program (HEAP), Supplemental Security Income (SSI), Medicaid, Veteran's Disability Pension (non-service related disability), Veteran's Surviving Spouse Pension (non-service related disability)

May need to show proof that you qualify for a Lifeline benefit using the following: A card or award letter to prove participation in one of these programs: Medicaid SNAP Certain other programs Pay stub/tax return to prove income is at or below 135% federal poverty guidelines Choose a participating company;Ask for a Lifeline application. Show at least one of these items: Unexpired ID such as a Driver's License Social security card Medicaid card Prior year's tax return Other documents that prove your identity Your company will process your application and may ask you to fill out additional forms. If approved, you will receive your Lifeline discount toward your bill. Your company can also apply your discount to an existing service.
