First Connections provides free, voluntary, confidential home visits to pregnant women and families with young children, birth to age three. Conduct home visits: Trained nurses, social workers, and community health workers meet with families in their homes to talk about their needs, answer questions they may have, and conduct comprehensive assessments.Link families with medical homes and social services: Home visits are based on the needs of the family. Visit topics may include health education and connections with appropriate healthcare services, human services, and community resources. If a child does not have a medical home, we can help the family find one. Visiting Nurse Home & Hospice can also conduct child wellness screenings and communicate the results with the child!s healthcare provider.

These groups address a variety of topics designed to help, heal and support those who have lost a loved one. These groups are open to the public and free of charge.

The monthly diabetes support group sponsored by the Visiting Nurse Services Home and Hospice is held at the Edward King Senior Center, 35 King St., Newport RI. The meeting is open to the public, and no appointment or referral is needed. Diabetes Support Groups are open to all those living with diabetes. The group is provided free of charge by Visiting Nurse Home & Hospice. Each meeting is facilitated by a certified diabetes educator. No registration or doctorâs referral is required. First Connections at Visiting Nurse Home & Hospice provides free, voluntary, confidential home visits in to pregnant women and families with young children. Conduct home visits: Trained nurses, social workers, and community health workers meet with families in their homes to talk about their needs, answer questions they may have, and conduct comprehensive assessments. If needed, home visitors can make arrangements for interpretation services during home visits. If you are an expecting parent, you may be contacted after the birth of your child to arrange a home visit. Engage families who may benefit from home visits. Link families with medical homes and social services: Home visits are based on the needs of the family. Visit topics may include health education and connections with appropriate healthcare services, human services, and community resources. If a child does not have a medical home, VNHH can help the family find one. Visiting Nurse Home & Hospice can also conduct child wellness screenings and communicate the results with the childâs healthcare provider. Hospice care is provided to patients who have advanced diseases that are considered incurable. Hospice care is about living each day to its fullest and helps to make a patientâs last months and days as comfortable and fulfilling as possibleâ¦making every moment count. Care is provided in the familiar surroundings of home or home-like setting. Care can also be provided in a long-term care setting or a hospital. The Hospice team focuses on patientsâ and their loved onesâ physical, emotional and spiritual comfort. The goal is for the patient and family to spend their remaining days with comfort and dignity. Hospice team visits regularly and is available to answer questions and provide support. The team helps prepare family and friends for the loss of a loved one, and help them deal with their grief through bereavement support groups.

Visiting Nurse Home & Hospice offers home care for women with early maternity discharge and post-partum complications. Services include: Breast feeding instruction and support; Wound care; Post partum care and monitoring for complications; Nutritional support; Assessment for maternal depression. Home care is provided to infants and children who have had surgery, an injury, or an illness. Services Included: Wound care; Cast care; Asthma care; Care for new diabetics; Care for seizure disorders; Medication administration; Physical, occupational, or speech therapies; Instruction in use of home medical equipment; Care for premature infants from Special Care Nurseries and Neonatal Intensive Care Units.

Description
Visiting Nurse Home & Hospice offers home care for women with early maternity discharge and post-partum complications. Services include: Breast feeding instruction and support; Wound care; Post partum care and monitoring for complications; Nutritional support; Assessment for maternal depression. Home care is provided to infants and children who have had surgery, an injury, or an illness. Services Included: Wound care; Cast care; Asthma care; Care for new diabetics; Care for seizure disorders; Medication administration; Physical, occupational, or speech therapies; Instruction in use of home medical equipment; Care for premature infants from Special Care Nurseries and Neonatal Intensive Care Units.
