Continuum Hospice was founded in 2008 with the mission to provide quality of life at the end of life. No request is too small, and no health need is too large for our specialized staff to address. Their care team provides an extensive collection of services including nurses, nursing, home health aides, durable medical equipment, medical supplies, medications and volunteers.

Eligibility:

Individuals are eligible for hospice care when a physician makes a clinical determination that life expectancy is six months or less if the terminal illness runs its normal course.

We train you, find you the right assignment and stand behind you as you grow. There are Two Types of Volunteers: Companion Volunteers - also known as Patient Care Volunteers, Companion Volunteers are part of a patient!€™s care team and Administrative Volunteers - administrative Volunteers do not work directly with patients and families.
