Age Details: 16-21, staffed apartment program; 13-21, outreach program and emergency shelter only for DCYF clients

Services: Operates scattered, multi-site staffed apartment programs for troubled adolescents in DCYF care; supervised independent living programs, outreach family intervention program and 45 day Evaluation Center. Offers Outpatient individual, group and family counseling; preparation for independent living, case management and community support services. Adoption/ foster care training and licensing, specializing in adolescents.

Income Source: Department of Children, Youth and Families.

Hours:

Mon-Fri 9:00AM-5:00PM, 24-hour emergency shelter.

Intake Process:

Referrals from Department of Children, Youth and Families (DCYF) or the Lead Agency of Communities for People!€™s (CFP) managed care networks.

Program Fees:

Counseling and Wellness program accepts third party billing through insurance. Birth, foster, and adoptive parents are not charged for any services. Foster care and adoption subsidies, including health insurance, are available for children in DCF, DYS and DCYF custody

Eligibility:

Children and adolescents in the custody Department of Children, Youth, and Families (DCYF).

Foster Parents: Adults who can provide a safe, stable and a loving home
