    Required Documents

    Letter of referral from school district, the child’s IEP, any pertinent evaluations, and a current audiogram.

    Eligibility

    Services specific to deaf and hard of hearing students between the ages of 3-21 years old.

    Intake Procedure

    Must have referrals from child's school. Most referrals start with a 45-day diagnostic placement. At the completion of the diagnostic period, the student’s educational team convenes to write an IEP for educational services.

        Description

        Child Outreach is a free service offered to all children 3 to 5 years old. Child Outreach provides information about your child’s development and resources that are available within your community. It can also help identify children who may need further assessment, intervention and/ or services at an early age to promote positive outcomes in kindergarten and beyond.
