Focuses on Cancer Care experience for patients, caregivers, and families. Screening and Diagnosis- Daymark works with your doctors to screen for and diagnose cancer. We facilitate testing that can tell you we it isn't cancer- and we help you take the next stops when it is. Social Support- we are experts at breaking down barriers to care. Our team can connect you with social support resources like transportation, financial, assistance, and housing. In Home Care- we bring care to you in your home so we can address symptoms and provide support where you feel most comfortable. Symptom Care- We help you manage the difficult symptoms of cancer and side-effects from treatment by phone, video, text, and in your home. Our team will also help you understand your prescribed medicines and guide you to the care you need. Care Navigation- Daymark manages appointments, coordinates follow-ups, and helps you understand your treatment plan. Think of us as your go-to guide and advocate. Our team of nurse practitioners, nurses, social workers, and health partners offers 24/7 support via phone, text, and email to help with urgent questions and care needs. Mental Health- Cancer is a life changing diagnosis that can impact all areas of your life, including mental health.
