Shared Living Program
Adults 18-64 with disabilities or anyone 65 and older who is eligible for Medicaid LTSS
Living in the same home as the caregiver who is at least 18 years old (excluding: spouse, legal guardian or power of attorney)
Needing help with some activities of daily living (ADLs and IADLs): bathing, eating, walking, toileting, dressing and transferring.
They help caregivers review eligibility and assist with the application process
Clients receive regular coaching with frequent contact
They connect caregivers to respite support to prevent burnout
They keep you informed about emerging issues to ensure timely interventions
High-quality, personalized care is delivered in person and through our secure, easy-to-use app
Care teams include a dedicated RN and care manager

Caregivers and care recipients may be eligible for our program if they meet the following criteria: The primary caregiver: Must be at least 18 years of age Cannot be the spouse, legal guardian and/or person legally responsible for the finances of the person receiving care The care recipient: Must be at least 18 years of age Must be eligible for Medicaid Must require help with one or more Activities of Daily Living, such as: Bathing Dressing Walking Transferring (helping get in/out of bed) Toileting Eating
